Delivery receipt
A signed acknowledgement that goods were delivered, and in what apparent condition.
Signed by the receiver on delivery, recording that the goods arrived and noting any visible damage or shortage.
Signing clean when the goods are visibly damaged undermines a later claim, which is why noting exceptions at the point of signature matters more than it appears to at the time.
